Dette er en pasientgruppe som ofte opps\u00f8ker en fysioterapeut og fort kan ende opp med diagnosen gluteal\u00a0<a href=\"https:\/\/www.fysionett.no\/tendinopati\/\">tendinopati<\/a>.<br \/>Dette er en tilstand som spesielt rammer kvinner over 40 \u00e5r. Og mange av dem har store problemer med de aller vanligste dagligdags gj\u00f8rem\u00e5l som \u00e5 reise seg opp, g\u00e5, g\u00e5 i trapper og noen sliter ogs\u00e5 med nattes\u00f8vnen da de ikke finner en god stilling. B\u00e5de ganglaget og ansiktsuttrykket hennes fortalte meg at dette var en dame som hadde smerter. Mye smerter.<br \/>Meget lykkelig over \u00e5 endelig ha funnet en ledig time p\u00e5\u00a0<a href=\"https:\/\/projects.techhivetechnologies.com\/lfiold\/timebestilling\/\">online bookingen<\/a>\u00a0s\u00e5 satte Mona seg ned i stolen lett andpusten og sa \u201ch\u00e5per at du er den riktige til \u00e5 hjelpe meg\u201d. Det var noe i m\u00e5ten hun sa det p\u00e5 som trykket p\u00e5 \u201cempati\u201d- knappen i meg, jeg fikk plutselig meget lyst til \u00e5 v\u00e6re \u201chan med den riktige kunnskapen\u201d. Men selv om jeg hadde 20 \u00e5rs bransjeerfaring som fysioterapeut s\u00e5 ble jeg klam i hendene og grepet av stundens alvor.<\/p><p>Var jeg \u201cden riktige\u201d eller skulle jeg rett og slett bare \u201cfeige ut\u201d gi henne en leksjon om kompleksiteten i menneskekroppen (ispedd noen filosofiske betraktninger, for \u00e5 \u201credde skinnet mitt\u201d) og \u201cta en tidlig kveld\u201d? Jeg viste jo at kj\u00f8ttkakene allerede stod klar p\u00e5 kj\u00f8kkenbenken der hjemme. Men det motsatte skjedde, rakrygget satt jeg der med nakken lett foroverb\u00f8yd med en mine som enhver professor ville ha misunnet meg, jeg skulle f\u00e5 med meg hver minste detalj.<\/p><p>Mona passet meget godt inn i bildet av en typisk hoftepasient. Over 40 \u00e5r gammel, sedat livsstil (regnskapsf\u00f8rer av yrke), overvektig, som endelig<br \/>hadde bestemt seg for gj\u00f8re noe med \u201cdet\u201d. Hun satt og tviholdt p\u00e5 et par Hooka-joggesko som et \u201cbevis\u201d p\u00e5 at hun hadde forst\u00e5tt alvoret og at endringen allerede var i gang. Det er mulig \u201cempati fr\u00f8et\u201d allerede ble s\u00e5dd der. Mona hadde hatt plagene i cirka 5 m\u00e5neder n\u00e5 og de hadde virkelig begynt \u00e5 t\u00e6re p\u00e5 nattes\u00f8vnen hennes den siste m\u00e5neden. \u201cJeg trodde smertene skulle g\u00e5 over\u201d, sa hun lett resignert.<\/p><p><strong>Potensielle mekanismer bak plagene<\/strong><br \/>Det er mulig jeg trengte Mona like mye som hun trengte meg. Jeg hadde v\u00e6rt hjemme 2 dager med en lei hals da navnet \u201c<a href=\"https:\/\/physiotec.com.au\/team_member\/dr-alison-grimaldi\/\">Dr Allison Grimaldi<\/a>\u201d dukket opp i et Googles\u00f8k jeg satt \u00e5 rota med i all kjedsomhet. Hun er en spennende australsk forsker som har tatt doktorgraden sin p\u00e5 hoftesmerter. Hun klarte \u00e5 fange oppmerksomhet min for \u201cmiddelaldrende overvektige damer med hoftesmerter\u201d.<\/p><p>Allison presenterte 2 hovedmekanismer som kan ligge bak tilblivelsen av denne typen hoftesmerter. Er du god i engelsk s\u00e5 kan du fordype deg\u00a0<a href=\"https:\/\/www.jospt.org\/doi\/full\/10.2519\/jospt.2015.5829#fig5\">her<\/a>.<br \/>Nummer 1: \u201cInnklemmings fenomenet\u201d. Tar du p\u00e5 utsiden av beinet ditt der hvor du tror hoften ligger, og f\u00f8ler et hardt bein da er du trolig p\u00e5 \u201ctrochanter\u201d (se bl\u00e5 pil p\u00e5 bilde 2). P\u00e5 dette beinet s\u00e5 fester det seg flere muskler, to av dem heter gluteus medius (glut med) og gluteus minimus (glut min). Den innerste delen kan bli klemt n\u00e5r beinet f\u00f8res innover (mot midtlinjen).<\/figcaption><\/figure><p>Normalt s\u00e5 utsettes en sene for stress i form av et drag (traksjon), men i dette tilfellet s\u00e5 utsettes de innerste fibrene i senen ogs\u00e5 for en innklemming (se bilde 2). Dette kan ogs\u00e5 skjer flere steder i kroppen (det vanligste er for eksempel akilles- og kne-senen) og noen ganger s\u00e5 kan den blir smertefull. B\u00e5de seneinnfestningen og omliggende strukturer kan bli p\u00e5virket (slimposer, ledd etc.)<\/p><p>Nr 2: \u201cDobbel-inneklemming av den ytre senen\u201d: n\u00e5r kroppen st\u00e5r med full vektb\u00e6ring p\u00e5 et bein s\u00e5 \u201cdras\u201d kroppen mot motsatt side pga skjevfordeling av vekten, dette skal glut med og<br \/>glut min (se bilde 3) motvirke. Men isolert sett s\u00e5 er de ikke sterke nok, de trenger hjelp av et ekstra lag som ligger utenp\u00e5. Disse best\u00e5r av en stor sene (iliotibiale senen) og i hovedsak 2 muskler, tensor fascie lata og gluteus maximus (\u00f8vre del). Det er tenkt at dette \u00f8ker trykket mot den innerste innfestningen til glut med som allerede har blitt inneklemt (og irritert).<\/figcaption><\/figure><p>Det var et lykketreff at Mona dukket opp med hoftesmerter, n\u00e5 fikk jeg sett om kunnskapen stemte i praksis. Vi var p\u00e5 en m\u00e5te avhengig av hverandre.<\/p><p><strong>Teori og praksis<\/strong><br \/>Mona sa: \u201cJo, jeg forst\u00e5r hva du sier, men hva gj\u00f8r vi?\u201d. N\u00e5r \u201calt gj\u00f8r vondt\u201d s\u00e5 har ofte pasienten resignert i \u00e5 pr\u00f8ve \u00e5 forst\u00e5 plagene sine. Hvor skulle hun begynne?<br \/>Hvis hovedbudskapet over er at jo n\u00e6rmere et bein er midtlinjen (eller over) jo mer drag f\u00e5r man p\u00e5 sener (p\u00e5 utsiden av hoften), s\u00e5 vil et bredere ganglag (beina fra hverandre) minske draget, ikke sant? En bevegelse av l\u00e5ret utover kalles abduksjon (minsker draget p\u00e5 muskel) og en bevegelse mot midtlinjen kalles adduksjon (minsker draget).<\/figcaption><\/figure><p>Vi studerte Mona n\u00e5r hun gikk. Det mest i\u00f8ynefallende var at hun gikk med kn\u00e6rne tett sammen (se bilde 1), beveget overkroppen fra side til side og haltet n\u00e5r hun gikk. \u201cTette kn\u00e6r\u201d kan v\u00e6re et tegn p\u00e5 svake muskler i hoften (glut med + min), det \u00e5 sette beinet mer \u201cunder seg\u201d er en kompensasjon for \u00e5 ikke v\u00e6re sterk nok i hoftemusklene og vil oppleves som en bedring av funksjonen (\u00f8kt drag av senene og muskler vil gj\u00f8re det enklere \u00e5 st\u00e5 imot belastningen).<\/p><p>Et vaggende ganglag (mot det vektb\u00e6rende beinet) brukes som en motvekt for manglende muskelkraft (pendelmotvekt). Det \u00e5 halte<br \/>er vanlig ved smerter eller kraftsvikt, Mona hadde nok begge deler. Hvis Mona var p\u00e5 kj\u00f8pesenter s\u00e5 kunne hun st\u00e5 \u00e5 \u201chenge\u201d p\u00e5 den ene hofta (i praksis \u00e5 f\u00f8re det vektb\u00e6rende beinet innover, se bilde 3),<br \/>dette b\u00f8r man unng\u00e5 (draget \u00f8ker betraktelig rundt hoften).<\/p><p>Hun hadde funnet noen \u201cgode r\u00e5d\u201d p\u00e5 nettet som gjorde at hun t\u00f8yde hoftene sine (i st\u00e5ende krysse beina og lene overkroppen til motsatt side) b\u00e5de morgen og kveld som igjen utsetter hoften for mye stress. Dette er noe hun ikke b\u00f8r gj\u00f8re p\u00e5 det n\u00e5v\u00e6rende stadiet. N\u00e5r hun satt p\u00e5 kontorstolen sin s\u00e5 hadde hun v\u00e6rt glad i \u00e5 krysse beina, som \u00f8ker draget p\u00e5 strukturene p\u00e5 utsiden av hoften (ugunstig \u00e5 gj\u00f8re n\u00e5). Hun hadde hele tiden trodd at hun sov best p\u00e5 siden, men v\u00e5knet opp flere netter med smerter. Det underste beinet blir presset innover og trykket mot trochanter \u00f8ker, det \u00f8vre beinet henger ofte foran det andre (draget blir stort p\u00e5 utsiden av hoften).<\/p><p><strong>Hvordan jobbet vi med Mona?<\/strong><br \/>Ved \u00e5 bevistgj\u00f8re funksjonelle arbeidsoppgaver og stillinger hvor senene rundt trochanter blir repetitivt- eller vedvarende- strukket gir oss et innblikk i hva som mest sannsynlig provoserer smertene. Dette \u00f8nsker vi \u00e5 unng\u00e5 p\u00e5 det n\u00e5v\u00e6rende tidspunktet. Dette var konkrete tiltak som var enkelt for Mona \u00e5 gj\u00f8re noe med. De vektb\u00e6rende stillingene er de f\u00f8rste man vil ta tak i \u00e5 endre, da disse ut\u00f8ver det st\u00f8rste trykket.
