Ca. hver 5. idrettsskade er et ankelovertr\u00e5kk. I enkelte ballidretter kan det utgj\u00f8re opp mot 40-50% av akuttskadene (Norsk helseinformatikk).<\/p><p>I denne bloggen skal vi se litt p\u00e5 hva som skjer i ankelen ved lette og moderate overtr\u00e5kk (skademaknismene). Hva b\u00f8r vi gj\u00f8re etter et overtr\u00e5kk? (Akutt behandling) Og til slutt skal vi se litt p\u00e5 hvordan man kan trene opp ankelen igjen og forebygge ny skade.<\/p><p><strong>Skademekanisme<\/strong><\/p><p>De aller fleste ankelovertr\u00e5kk skjer ved at foten vris innover n\u00e5r man f.eks. tr\u00e5kker p\u00e5 en rot eller stein, eller ved at man lander forkj\u00e6rt under en idrettsaktivitet med l\u00f8p eller hopp. Foten kan ogs\u00e5 vris utover, men dette er ikke s\u00e5 vanlig.<\/p><p>P\u00e5 utsiden av ankelen er det 3 leddb\u00e5nd. Et fremre (lig. talofibulare anterius), et midtre (lig.calcaneofibulare) og et bakre (lig.talofibulare posterius). Disse har til hensikt \u00e5 stabilisere ankelen. B\u00e5ndene blir strukket og et eller flere av de kan ryke helt eller delvis. P\u00e5 fagspr\u00e5ket blir dette kalt total eller partiell ruptur av ligamentene. Som regel er det det fremre b\u00e5ndet som ryker f\u00f8rst, evt. i kombinasjon med det midtre. En sjelden gang ryker alle tre, men da snakker vi om en mer alvorlig og kompleks ankelskade.<\/p><p>P\u00e5 innsiden har vi et bredt leddb\u00e5nd (lig.deltoideum) som kan bli skadet hvis foten vris utover.<\/p><p>Det er blod\u00e5rer i leddb\u00e5ndene og vi f\u00e5r en bl\u00f8dning i vevet.<\/p><p>Diagnosen stilles etter unders\u00f8kelse hos lege eller fysioterapeut. Det b\u00f8r tas et r\u00f8ntgen ved mistanke om brudd.<\/p><p><strong>Akutt behandling<\/strong><\/p><p>I og med at vi f\u00e5r en bl\u00f8dning i vevet er m\u00e5let med akutt behandlingen (24-48 timer) \u00e5 begrense denne bl\u00f8dningen og hevelsen som vil oppst\u00e5 rundt ankelen. Behandlingen f\u00f8lger POLICE- prinsippet for akuttbehandling (protect, optimal load, ice, compression, elevation). Jeg synes det er spesielt viktig at man avslutter aktiviteten og f\u00e5r p\u00e5 en kompresjon s\u00e5 fort som mulig. Tidlig stopp av bl\u00f8dningen vil gj\u00f8re opptreningen enklere og f\u00f8re til en raskere tilbakegang til normal aktivitet. I enkelte tilfeller kan avlastning med krykker v\u00e6re aktuelt i noen dager.<\/p><p>Prinsippene for akuttbehandling har v\u00e6rt i stadig utvikling de siste \u00e5rene (ice-rice-price-police). Det siste forslaget som n\u00e5 er blitt lansert er PEACE &amp; LOVE som du kan lese mer om\u00a0<a href=\"https:\/\/projects.techhivetechnologies.com\/lfiold\/blogg\/peace-love\/\">her<\/a>\u00a0(tidligere blogginnlegg).<\/p><p><strong>Opptrening og forebygging<\/strong><\/p><p>M\u00e5let i tidlig opptreningsfase (de f\u00f8rste dagene) er \u00e5 gjenvinne normal, \u00absmertefri\u00bb bevegelighet\/funksjon og \u00e5 bli kvitt hevelsen. Jeg pleier \u00e5 anbefale ergometersykkel som en start\u00f8velse hvor man kan bevege uten s\u00e6rlig belastning. Ellers anbefales bevegelighetstrening innenfor det som kjennes \u00abgreit\u00bb ut. Kompresjon kan brukes etter trening for at ikke ny bl\u00f8dning og mer hevelse oppst\u00e5r.<\/p><p>Etter hvert er m\u00e5let \u00e5 normalisere gange\/l\u00f8p. F\u00f8rst rett frem, deretter med retningsforandringer. Skal du tilbake til en spesiell aktivitet, s\u00e5 etterlign bevegelsene i aktuell aktivitet. Man b\u00f8r starte opp med balanse (leddsans) og koordinasjonstrening. Her kan man f.eks. bruke balansepute\/brett, et h\u00e5ndkle e.l. for \u00e5 skape ut ustabilt underlag. Start enkelt med gradvis progresjon. Et regime er 10 minutter, 5 dager i uka, i 10 uker. Jeg pleier ogs\u00e5 \u00e5 anbefale t\u00e5hev og kneb\u00f8y for bl.a. \u00e5 ivareta styrken i beinet. P\u00e5 bl.a.\u00a0<a href=\"http:\/\/skadefri.no\/\">skadefri.no<\/a>\u00a0ligger det treningsprogram for ankel.<\/p><p>Etter et lett til moderat ankelovertr\u00e5kk p\u00e5 utsiden kommer man seg ofte raskt og de fleste er tilbake i full trening eller hverdagslig aktivitet etter en til noen f\u00e5 uker.
