Innen mitt fagfelt kan vi nok trygt fastsl\u00e5 at dette er en diagnose som har v\u00e6rt under-rapportert som viktig medvirkende \u00e5rsak til mange muskel-skjelettplager opp gjennom \u00e5ra. I dette blogg-innlegget skal jeg ta for meg en \u00abdiagnose\u00bb som tvert imot er over-rapporter som \u00e5rsak til mange muskel-skjelettplager: Kroppsholdningen og sittestillingen v\u00e5r har f\u00e5tt (og f\u00e5r fortsatt) mye av skylden for alskens \u00abvondter\u00bb, s\u00e6rlig ryggplager, og blir hyppig brukt som \u00e5rsaksforklaring.<\/p><p>Kroppsholdning er et tema som stadig tas opp av pasienter p\u00e5 mitt kontor, diskuteres mellom kollegaer, i media og i samfunnet for\u00f8vrig. En vanlig oppfatning er at man f\u00e5r ryggplager av \u00e5 sitte eller st\u00e5 med \u00abd\u00e5rlig holdning\u00bb, eller n\u00e5r vi \u00abl\u00f8fter feil\u00bb. Til tross for at det ikke finnes dokumentasjon for disse p\u00e5standene, lever de i beste velg\u00e5ende, ikke bare hos mannen i gata, men ogs\u00e5 innad i helsevesenet. I tillegg tjener en hel \u00abholdnings-industri\u00bb seg rik p\u00e5 produkter og intervensjoner som hevder \u00e5 korrigere holdning og forebygge smerte.<\/p><p>I august-utgaven av Journal of Orthopedic Sports Physiotherapy kunne vi lese artikkelen \u201cSit up straight; Time to re-evaluate\u201d (1), som oppsummerer det forskningen sier om kroppsholdning pr dags dato. Jeg vil videre gjengi hovedpunktene fra denne artikkelen:<\/p><p><strong>1.<\/strong>\u00a0<strong>Det finnes ingen korrekt holdning.<\/strong><br \/>Til tross for hva mange tror, er det ingen sterk evidens for at en optimal holdning finnes eller at \u00e5 unng\u00e5 \u00abd\u00e5rlig holdning\u00bb vil forebygge ryggsmerter.<\/p><p><strong>2. Ulike kroppsholdninger er helt naturlig<\/strong>.<br \/>Det er naturlige variasjoner i ryggens kurvatur og det finnes ikke en spesiell kurvatur som er mer assosiert med smerter. Smerte b\u00f8r ikke kobles til normal-variasjoner i ryggens kurvatur.<\/p><p><strong>3. Kroppsholdning reflekterer tanker og hum\u00f8r.<\/strong><br \/>Kroppsholdning kan gi innsikt i en persons sinnsstemning, f\u00f8lelsesliv og kroppsbilde. Noen holdninger inntas som del av en beskyttelsesstrategi og kan reflektere bekymringer rundt kroppens s\u00e5rbarhet. \u00c5 forst\u00e5 at det er ulike \u00e5rsaker til foretrukne kroppsholdninger kan v\u00e6re nyttig.<\/p><p><strong>4. Det er helt trygt \u00e5 innta mer komfortable stillinger og disse varierer mellom individer.<\/strong><br \/>\u00c5 utforske ulike stillinger, inkludert de man ofte unng\u00e5r, og \u00e5 endre typiske \u00abvane-stillinger\u00bb kan gi symptomlette.<\/p><p><strong>5. Ryggraden er robust og t\u00e5ler mye.<\/strong><br \/>Ryggraden er en robust og tilpasningsdyktig struktur som trygt kan beveges og belastes i et utall ulike posisjoner. Advarsler om \u00e5 beskytte ryggen er vanlig i samfunnet, men st\u00f8ttes ikke av forskning og kan tvert imot f\u00f8re til frykt.<\/p><p><strong>6. \u00c5 sitte er ikke farlig.<\/strong><br \/>\u00c5 sitte i mer enn 30 minutter i en og samme stilling er ikke farlig. Generell bevegelse og \u00e5 endre stilling kan imidlertid ofte hjelpe, og \u00e5 v\u00e6re fysisk aktiv er generelt viktig for helsen.<\/p><p><strong>7. \u201cOne size does not fit all\u201d.<\/strong><br \/>Unders\u00f8kelser av kroppsholdning- og bevegelsesm\u00f8nster forebygger ikke ryggplager (p\u00e5 arbeidsplassen eller ellers). Foretrukne l\u00f8fteteknikker avhenger av de naturlige variasjonene i ryggradens kurvatur, og r\u00e5d om \u00e5 tilpasse seg en spesiell stilling eller \u00e5 \u00abstive av ryggen\u00bb har ingen st\u00f8tte i forskning.<\/p><p>At for mye stillesitting, som i for lite fysisk aktivitet, er et samfunnsproblem som gir mange negative helseeffekter vet vi i dag veldig godt. Men p\u00e5 hvilken m\u00e5te du sitter n\u00e5r du f\u00f8rst sitter er ikke noe vi hverken kan eller b\u00f8r generalisere.
